Shift: First Shift (United States of America) – Care Transition RN – Monday to Friday 8:00 AM – 5:00 PM.

Responsible for coordinating a patient‑centered, interdisciplinary plan of care for patients in the acute inpatient care setting. Work collaboratively with internal and external care providers, facilities, and agency partners to ensure a coordinated post‑acute discharge or treatment plan is in place to support a seamless transition of care. Responsible for implementing workflows and processes involved in facilitating discharge care planning activities such as arranging for post‑acute facility transfers or in‑home support services.

Required Qualifications
  • Minimum of three years nursing experience.
  • Bachelor’s degree in nursing (BSN).
  • Effective verbal and written communication skills.
  • Ability to prioritize and make independent clinical judgments.
  • Experience with basic patient care equipment, policy, procedure, and protocol manuals.
  • Ability to work collaboratively and cooperatively with an interdisciplinary team.
  • Effective critical‑thinking/analytical skills.
  • Ability to effectively evaluate care across the continuum.
  • Currently licensed as a Registered Nurse in the state of Nebraska or a current compact state license.
  • Basic Life Support (BLS) certification required from an American Heart Association (AHA) endorsed program (effective for new hires starting January1,2023 or later).
Preferred Qualifications
  • Certification preferred in Advanced Cardiac Life Support (ACLS), Blood and Marrow Transplant Certified Nurse, Certified Diabetes Educator (CDE), International Board Certified Lactation Consultant (IBCLC), Oncology Nursing Certification (OCN), Oncology Nursing Society Chemotherapy and Biotherapy, Pediatric Advanced Life Support (PALS), transplant coordination, and/or Trauma Nursing Core Course (TNCC).
  • Prior nursing experience in related field (oncology, cardiovascular care, neonatal intensive care, nephrology, solid organ transplantation, general medical/surgical care, emergency care, critical care, med spa, dialysis, interpretation, explanation of physical/laboratory/screening results).
  • Master's of Science in Nursing (MSN) degree.
  • Successful completion of a nephrology nurse education program.
  • Knowledge of health‑care financial environment and reimbursement systems.
  • Experience with Microsoft Office applications including Word and Excel.
  • Experience with graphic software.
  • Experience with patient teaching materials.
  • Understanding of related patient registries and databases.
  • Experience leading/supervising a team.
