Duties

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. The Registered Nurse (RN) - Case Management (CM) Oncology/Hematology is a clinical expert who uses advanced specialized knowledge and judgement for delivery of high quality and evidence-based care to oncology patients and their families.

Provides patient management, care coordination, and disposition planning for cancer patients. This includes, but is not limited to clinic preparation including obtaining outside medical records and pathology specimens from outside facilities, and consult management. Additional duties of the Registered Nurse (RN) - Case Management (CM) Oncology/Hematology include, but are not limited to:

  • Coordinates comprehensive care and resources for Veterans and their families by collaborating with VA and community providers to ensure continuity of care throughout the diagnostic and treatment continuum.
  • Conducts ongoing assessment and oversight of patient care plans, ensuring timely scheduling, completion of interventions, and effective tracking of treatment progress.
  • Participates in interdisciplinary care management to address the medical, nursing, and psychosocial needs of patients and caregivers, contributing a professional nursing perspective to team discussions.
  • Triages oncology-related symptoms and adverse events, collaborating with the interdisciplinary team to formulate and adjust care plans based on current best evidence and patient needs.
  • Ensures accurate documentation and compliance with established policies by completing physiological assessments, maintaining electronic health records, and participating in continuous quality improvement activities.
  • Clinic intake including obtaining vital signs, performing EKGs, blood glucose testing, administering vaccines, completing the required clinical reminders, oncology distress screening, and the outpatient clinic intake note.
  • Rooming patients and giving a warm hand-off to providers to ensure a smooth clinic flow.
  • Arrangement and assistance with procedures
  • Bone Marrow Transplant Coordination
  • Survivorship Care Plans
  • Survivorship 101 Support Group Collaboration
  • Active Participation in Tumor Board and Cancer Committee
  • 6 hours of oncology-related continuing education nursing contact hours per calendar year or Oncology Nursing Certification in accordance with our Commission on Cancer Accreditation Required.
