Our Hospice Nurse Care Manager (RN) Will:
  • Provide individualized care by treating each patient as they would want a loved one treated.
  • Develop care plans that reflect each person and their needs.
  • Be detail-oriented, knowing that carefully managed patients (medications, treatments, communication, aide supervision, etc.) have more comfort and dignity.
  • Focus on customer service when interacting with patients, families, communities/facilities, and all partners and co-workers.
  • Help team members improve through instruction and support.
  • Build and manage meaningful relationships with all partners.
Our Hospice Nurse Care Manager (RN) Has:
  • An active state-required RN license or a compact state license.
  • 2+ years of hospice experience.
  • An active Driver's License with a reliable vehicle, insurance, and an acceptable driving record.
  • The ability to meet the physical requirements for patient care with reasonable accommodations (e.g., Independently lift at least 30lbs).
  • Humility to know that even experts learn something new every single day.
  • A positive attitude and desire to make every day better for the people they encounter.
