One full-time position | Located on the main Livestock Exchange Campus (30th & L) | Monday - Friday 8:00 am - 5:00 pmJob Duties The Social Worker performs a variety of social work services within commonly accepted standards of social work practice, ranging from simple to crisis cases. The Social Worker assesses the needs of patients and their families and develops appropriate programs/services.

The Social Worker investigates and determines the kinds of services needed by an individual client and/or family; carries out approved treatment plans; prepares case records and reports; and performs other related duties as assigned. The position requires a working knowledge of the application of principles, methods, and techniques of the social work field to the solution or alleviation of problems of individuals and/or families.

Member of an integrated health care team:
The Social Worker uses effective interpersonal skills in managing complex interactions involved in the functioning of a multidisciplinary team. Serves as a resource to clinical staff in managing complex psychosocial issues and accessing community resources. Creates resources for the integrated health care team based on the needs of their patients.

Patient Care:
Applies the principles, practices, and codes of ethics of social work services in the performance of assignments such as assessing, developing, and providing direct services or programs to individuals, families, and/or groups. Assists individuals and/or families to find satisfactory ways of coping with and understanding the nature and extent of their problems in their social situations and relationships. Assures that patients and families are knowledgeable of available community resources and how to access them.

Provides assistance with medication, transportation, housing, and other social determinants of health. Point of contact for patients who present with any type of abuse or neglect. Conducts screenings and education for patients related to their health and well-being.

Patient Advocate:
Functions as an advocate for patient rights and organizational ethics. Possesses a strong commitment to diversity, equity, and inclusion; community engagement; and social justice.

Case Management:
Identifies and assesses available services to meet the social, financial, and emotional needs that affect a patient's medical condition, treatment, recovery, and transition to a lower level of care. Maintains an ongoing current file of community resources and programs. Assists patients with formal and informal referrals, phone calls, and completion of paperwork and applications.

Outreach Services:
Acts as a liaison between One World and community resources. Networks with external organizations and groups with the goal of creating collaborations and partnerships that benefit One World patients. Conducts internal and external presentations as needed.

Supervisor:
Provides orientation, training, and supervision to social/human services practicum students.

Must have a Bachelor’s Degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Certified Social Worker (CSW) by Nebraska State Law is preferred upon hiring. If not certified at the time of hire, certification will be required within 90 days of employment.

Excellent verbal and written communication skills;
English/Spanish proficiency required.

Task-oriented and detail-oriented with excellent customer service and organizational skills.

Great computer proficiency with Microsoft Office and a willingness to learn other programs.

Working knowledge of the Latino culture and commitment to cultural competence.

Familiarity with social service resources in the community.

Willingness to complete home visits as deemed necessary (Motor Vehicle Report (MVR) required) and a valid driver’s license.
