We are seeking an experienced and committed Advocate to join our dynamic and innovative organization. The Mental Health Family Advocate plays a vital role in connecting children and families to trauma‑informed mental health and advocacy services.
This position integrates elements of family advocacy, case coordination, and mental health navigation to ensure families receive timely, appropriate, and compassionate care. The Advocate collaborates closely with referral sources, clinicians, and community partners to remove barriers, engage families, and strengthen participation in services. This position supports both internal referrals from Project Harmony programs and community referrals, ensuring each family receives coordinated, culturally responsive, and strength‑based support.
Duties And Responsibilities
- Conduct Intake interviews and screening calls to gather demographic, psychosocial, and background information
- Evaluate client readiness for mental health services and connect families to appropriate Project Harmony or community-based providers
- Coordinate and schedule initial appointments, ensuring timely engagement and completion of pre‑service documentation
- Complete relevant screeners or assessments to identify client needs, strengths, and risk factors including crisis assessment and safety planning
- Use a strengths‑based approach to align families with the most suitable therapeutic or support services
- Provide trauma‑informed, strengths‑based support to children, youth, and caregivers during times of crisis and transition
- Offer psycho educational resources to help families understand mental health needs, treatment options, and community resources
- Facilitate or support psycho educational groups for parents and caregivers, promoting resilience and connection
- Partner closely with Project Harmony clinicians, case managers, and program staff to ensure families receive wraparound support
- Participate in multidisciplinary team (MDT) meetings, providing updates and insights regarding family progress and service needs
- Collaborate with community providers, schools, and referral partners to support shared clients and remove barriers (transportation, childcare, financial, emotional) that impact service engagement and help families develop solutions throughout the tenure of the case
- Maintain active communication with therapy providers to support engagement, progress, and care planning
- Enter accurate and timely documentation into electronic health record (EHR) platform and case management systems
- Track and report data on referrals, engagement, and outcomes to support program evaluation
- Uphold confidentiality, HIPAA, and ethical standards in accordance with Project Harmony policies and state regulations
- Deliver services with empathy, respect, and inclusivity—honoring every family’s background and lived experience
- Actively participate in supervision, team meetings, and ongoing professional development to enhance quality services for children and families served
- Ensure culturally competent services are provided to all families we serve without regard to race, ethnicity, religion, socioeconomic status, disability, gender or sexual orientation
- Other duties as assigned
Education
- Bachelor’s degree in Social Work, Psychology, Human Services required, or a related field
- Training in trauma‑informed care, child welfare, or behavioral health systems strongly preferred
Experience and Training
- Experience in child welfare preferred
- Experience and/or training in crisis intervention preferred
- Knowledge of community mental health resources
Knowledge/Skills/Abilities
- Must be Bilingual in Spanish (speaking, reading and writing).
- Good working knowledge of community resources
- Strong communication and interpersonal skills with the ability to build rapport with diverse clients and partners
- Excellent time management, organization, and documentation skills
- Ability to collaborate effectively across systems and disciplines
- Demonstrate proficiency in professional verbal and communication skills
