Stephen Center has served homeless and low-income individuals in Omaha since
1984 with the mission to partner with the community, families and individual to overcome homelessness, addiction and poverty. The Pettigrew Emergency Shelter and Permanent Supportive Housing offerings provide safe and secure housing while individuals and families complete an assessment of permanent housing options. All Stephen Center residents have access to dual diagnosis support services via the HERO addiction/recovery program. 100% of Stephen Center shelter supportive housing participants are living in poverty.
Housing transition support identifies poverty barriers that may impact one’s ability to achieve or maintain permanent housing through case management, life skills, and behavioral health services. Stephen Center serves approximately 250people daily in all programs.
The HERO Case Manager plays a critical role in supporting a coordinated, client-centeredrecovery environment by integrating clinical, operational, and community-based services. Working collaboratively across interdisciplinaryteams, this position ensures safety, accountability, and continuity of carefor individuals navigating co-occurring substance use and mental healthrecovery. The role focuses on connecting clients to appropriate referrals and resources that support both immediate needs and long-term recovery, whilepartnering with clinical staff, leadership, and administrative teams to maintain a safe and structured program environment.
Through ongoing assessment, the Case Manager identifies client needs related to physical andmental health, housing, employment, family support, and other essential services, and works closely with the HERO care team and community partners to implement individualized solutions. This position supports clients inpreparing for independent living while ensuring all services, clientengagement, and outcomes are accurately and timely documented in alignment with program standards, compliance requirements, and overall quality of care.
KeyAccountabilities
- Maintain a safe, structured, and therapeutic environment by consistently following all safety, security, and program guidelines, including class participation, behavior compliance, andclient activity as needed.
- Facilitate and coordinate dailyrecovery programming, including groups, meetings, and structured activitiesaligned with the HERO continuum of care (STR, IOP, OP).
- Complete comprehensive intakes andclient orientations, ensuring everyone is welcomed into the program withclear expectations, resources, and support.
- Develop individualized service planswithin 24 hours of admission, in collaboration with clinical staff and leadership, ensuring plans are person-centered, goal-oriented, and responsive to client needs.
- Implement and manage service plans through ongoing engagement with clients, interdisciplinary teams, and community providers; documenting services, progress, barriers, and outcomesin a compliant manner.
- Ensure all documentation is entered into the Electronic Health Record within 72 hours of service delivery,maintaining accuracy, compliance, and alignment with agency and regulatory standards.
- Coordinate access to medical, dental,psychiatric, and community-based services, strengthening partnerships tosupport whole-person care and successful transitions.
- Support high-acuity anddual-diagnosis clients’ coordination of care to reduce attrition and enhanceretention.
- Assist clients in navigating system sand completing applications related to healthcare, income, housing, and othersupports necessary for successful community reintegration.
- Participate actively ininterdisciplinary team meetings, contributing to case consultations, care planning, and shared problem-solving to ensure continuity and quality ofcare.
- Support program operations through daily responsibilities including room checks, chore verification, medicationobservation, and reinforcement of program expectations and clientaccountability.
