Rehabilitation Specialist - Assertive Community Treatment (Nebraska)

Omaha, NE, USA
Compensation: between $40,800 and $51,616 per year (salary is determined by total years of relevant experience)
Work Schedule:
40 hours per week (Monday through Friday, 8:30 a.m. to 5:00 p.m.)
- Bachelor’s Degree in Human Service field.
- Three years in case management, vocational or skill development experience.
Professionals at this level have excellent communication skills with both client and community institutions. Specific project, program or office responsibilities may occur at this level. Decision-making, problem solving, and highly developed interpersonal skills are critical. This position functions as a fully integrated Team Member of the Assertive Community Treatment Team, which is a multi-disciplinary team that assists individuals with a serious mental illness to minimize symptoms and the resulting impairment in daily functioning.
Essential Duties and Responsibilities- Client assessment of individual skills/interests and formulation of a plan for employment/education goals.
- Assist clients in developing skills in the areas of daily living, social and interpersonal relationships, and occupational capabilities.
- Provide support and consultation to team members and community partners.
- Maintain all contact documentation and comply with contract conditions.
- Provide problem solving and crisis intervention as needed.
- Coordinate services with other community agencies and resources.
- Provide service by helping clients manage their finances.
- Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the agency.
- Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
- Is dependable and punctual regarding scheduling and attendance.
- Abides by all specific program and Agency procedures, policies, and requirements.
- Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
- Performs other program related duties as assigned.
- Create, maintain and share as appropriate a dynamic self-care plan.
- Strive to make connections between the agency and the larger community whenever possible in order to contribute to the agency’s ongoing fundraising and friend-raising efforts.
- Essential functions of this job are to be performed on company physical work site or designated workspace.
