HERO Licensed Social Worker

Stephen Center has served homeless and low-income individuals in Omaha since
1984 with the mission to partner with the community, families and individuals to overcome homelessness, addiction and poverty. The Pettigrew Emergency Shelter and Permanent Supportive Housing offerings provide safe and secure housing while individuals and families complete an assessment of permanent housing options. All Stephen Center residents have access to dual diagnosis support services via the HERO addiction/recovery program. 100% of Stephen Center shelter supportive housing participants are living in poverty.

Housing transition support identifies poverty barriers that may impact one’s ability to achieve or maintain permanent housing through case management, life skills, and behavioral health services. Stephen Center serves approximately 250people daily in all programs.

Purpose:

The Licensed Social Worker serves a critical role within the HERO Program, providing comprehensive inpatient and outpatient behavioral health treatment services to individuals with co-occurring substance use and mental health disorders. This position is responsible for conducting clinical assessments, developing and implementingindividualized treatment plans, facilitating individual and grouppsychotherapy, and delivering both clinical and psycho educational programming on a daily basis. The therapist supports continuity of care across all levelsof programming (STR, IOP, and OP) while maintaining accurate and timelyclinical documentation in accordance with regulatory standards.

The Licensed Social Worker is expected to function within a structured,team-based environment, actively engaging in interdisciplinary collaboration and balancing clinical judgment with program expectations. Services must alignwith evidence-based practices, CARF standards, and all applicable federal,state, and grant requirements. This role is a key contributor to programculture, client engagement, and overall treatment outcomes, with clear expectations for consistency, accountability, and professionalism in maintaining a safe, therapeutic, and recovery-oriented environment.

K ey Accountabilities:

Service Delivery & Clinical Responsibilities

• Provide comprehensive, evidence-based behavioral health services to individuals with co-occurring mental health and substance use disorders.
• Conducttimely and accurate mental health and substance use assessments in accordance with program and regulatory standards.
• Develop,implement, and regularly update individualized treatment plans with measurable short- and long-term goals in collaboration with the interdisciplinary team.
• Facilitateindividual, group, and family counseling sessions, including daily clinical and psycho educational groups.
• Conductsubstance abuse assessments, drug and alcohol evaluations, and chemical dependency evaluations in accordance with clinical, state, and program requirements, within the scope permitted by licensure.
• Maintaina mixed caseload while ensuring continuity of care across all programlevels (STR, IOP, OP, Aftercare).
• Continuously assess client progress and adjust treatment interventions based on clinical need and team collaboration.
• Provideonsite crisis intervention and participate in an on-call rotation forafter-hours client emergencies.
