Family Support Worker

OHB Omaha Home for Boys seeks a Family Support Worker to empower youth and families through technology-enabled services. In this role, you will coordinate care plans, connect families with community and digital resources, and track progress using innovative software platforms. You'll collaborate with clinical, education, and tech teams to deliver data-informed support, coach caregivers on digital tools, and maintain accurate documentation.

Responsibilities

• Provide direct support and coaching to youth and families using trauma-informed practices
• Conduct intake, assessment, and ongoing evaluation of family needs and goals
• Develop, implement, and monitor individualized family support and service plans
• Use case management and reporting software to document services and track outcomes
• Coordinate referrals and link families to community and digital resources
• Collaborate with clinical, educational, and technology teams to align services
• Maintain accurate, timely, and confidential records in compliance with policies
• Provide outreach, home or virtual visits, and follow-up to ensure continuity of care
• Participate in team meetings, trainings, and continuous improvement initiatives
• Use data and software tools to identify trends and adjust support strategies
