Summary/PrimaryPurpose:
Provide
in-home rehabilitative service for Veterans with the intention of supporting the individual to maintain community living and interventions. Assist in developing services necessary to maintain community living and prevent exacerbation of illness and admission to higher levels of care.
Essential Duties And Responsibilities- Complete comprehensive assessment and develop Individual Program/Service Plan (ISP) within 7 days of admission.
- Maintain clinical supervision of ISP and ongoing assessments including risk assessments, safety contracts, self-responsibility plan, Global Appraisal of Individual Needs (GAIN) screening, and update ISP every 90/180 days.
- Must have a minimum of three face-to-face contacts per month or more frequently as needed.
- Provides support interventions with focus on daily living skills, education, budgeting, medication compliance and self-administration, relapse prevention, social and independent living skills.
- Develop and implement strategies to ensure the individual becomes engaged and remains engaged in mental health treatment services, as recommended and included in the treatment/recovery plan.
- Perform services and case management activities in conjunction with the Veterans Administration Liaison.
- Maintain case records, statistics and assist with reports and manuals, when needed.
- Complete all required case file documentation including but not limited to ISPs, case notes, intake and discharge paperwork, needs assessments, and any other required documentation.
- Participate with the treatment/rehabilitation team on individual’s progress and response to community support interventions in the areas of relapse prevention, substance use/abuse, application of education and skills, and the recovery environment
- Prepare and implement crisis/relapse plans. Respond rapidly to crisis situations, including a comprehensive assessment and thorough risk assessment to transition consumer’s return to community and avoid need for higher level of care.
- Facilitate communication between treatment and rehabilitation providers and primary/supervision practitioners.
- If hospitalization or residential care is necessary, facilitate the individual’s transition back into the community upon discharge in cooperation with the treatment provider.
- Present program participants at intake, at time of extension, and to Multi-Disciplinary Team regarding feedback to address any barriers or clinical issues.
- Provide on call coverage rotation including taking calls via telephone and going into the office to address a situation if warranted.
- Driving is an essential function of the position.

Associates Degree (A.S.) Required from a two-year College or university in psychology, social work, or related field or, in another field with advanced education in psychology, social work, sociology or related field.
