Greenfield Rehabilitation Agency is expanding and seeking passionate Physical Therapists (PTs) to join our exceptional therapy team supporting senior communities in Onalaska, Wisconsin

- Conduct initial and ongoing evaluations of residents’ mobility, strength, balance, and endurance
- Identify barriers to functional independence and design individualized physical therapy programs
- Address challenges related to mobility, posture, gait, and injury or illness recovery
- Provide interventions to help residents regain strength, prevent falls, and maintain independence
- Support residents in daily functional tasks, recommending adaptive equipment as needed
- Conduct exercises and mobility programs to enhance coordination, flexibility, and overall physical function
- Assess living spaces to recommend modifications for safety and accessibility
- Educate residents, families, and staff on therapy goals, safe mobility, and fall prevention
- Collaborate with OTs, SLPs, nursing, and interdisciplinary teams to ensure holistic care
- Maintain accurate records of assessments, treatments, and progress, ensuring compliance with healthcare regulations and community policies

Qualifications- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
