Location: 321 Genesee St, Oneida, NY 13421
Schedule: 3x12 Nights | 36 Hours/Week
Shift: 7:00 PM – 7:30 AM | Every Other Weekend | Holidays as Required
Oneida Health is seeking an experienced Licensed Practical Nurse (LPN) to provide compassionate, high-quality care to residents in the Rehabilitation & Extended Care Facility. The LPN will collaborate with physicians, registered nurses, certified nursing assistants, unit managers, and other interdisciplinary team members to ensure residents receive safe, effective, and individualized care.
Key Responsibilities- Provide compassionate, resident-centered nursing care.
- Monitor residents and promptly report changes in condition to the RN and physician.
- Collaborate with the interdisciplinary care team to develop and implement care plans.
- Administer medications and treatments as ordered.
- Assist with wound care and other nursing procedures.
- Delegate and supervise assigned support staff to ensure quality care.
- Order medications and medical supplies as needed.
- Maintain accurate documentation and enter physician orders into the EMR.
- Coordinate resident appointments and communicate with families regarding appointments and transportation.
- Participate in care conferences, discharge planning, and departmental meetings.
- Maintain compliance with facility policies, medication management, and documentation standards.
- Report emergencies, safety concerns, and incidents involving suspected abuse, neglect, mistreatment, or exploitation.
- Promote a clean, safe, and healthy environment for residents.
Education
- High School Diploma or equivalent.
- Graduate of an accredited Licensed Practical Nurse (LPN) program.
Licensure & Certifications
- Active, unencumbered New York State LPN License (Required).
- BLS/CPR Certification (Preferred).
- Minimum 2 years of LPN experience (Required).
- Strong clinical and nursing judgment.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Strong teamwork and collaboration skills.
- Compassionate, patient-focused approach to care.
- Basic computer and EMR documentation skills.
- Ability to lift, carry, push, and pull 45 lbs or more.
- Ability to stand, walk, and sit for extended periods.
- Frequent bending, kneeling, reaching, and repetitive movements.
- Good hand-eye coordination.
- Ability to operate a motor vehicle if required.
