Registered Nurse (RN) - LTAC

The LTAC RN provides ongoing care for medically complex adults in a long-term acute care setting. The role centers on closely monitoring patients, coordinating with an interdisciplinary team, and supporting safe discharge planning. The nurse collaborates with physicians, therapists, and care managers to optimize recovery and manage complex conditions.

In this role you will work within a hospital-affiliated LTAC unit, using specialized assessment and documentation to observe trends and respond to clinical changes as part of a dedicated care team.

Responsibilities
  • Assess and monitor patient status, including vital signs, labs, and response to therapies.
  • Administer medications and treatments per orders and manage IV therapy and infusions.
  • Perform wound care, dressing changes, and ostomy care as indicated.
  • Monitor for complications and escalate promptly according to protocol and physician orders.
  • Collaborate with physicians, case managers, therapists, and families to develop and adjust comprehensive care plans.
  • Document findings, interventions, and patient progress accurately in the electronic health record.
  • Educate patients and families on care plans, medications, and discharge expectations.
Qualifications

Education:

Associate degree in Nursing or Bachelor of Science in Nursing from an accredited program.

Licensure:

Current and valid RN license in the state of practice.

Certifications:

  • Basic Life Support (BLS) certification for Healthcare Providers
  • Advanced Cardiovascular Life Support (ACLS) recommended for ongoing critical care knowledge
  • Either a specialized LTAC or critical care related certification is preferred

Experience or Skills:

Strong assessment, clinical judgment, and documentation skills; proficiency with IV therapy, wound care, and telemetry as applicable; ability to work in a multidisciplinary team and support discharge planning.

Experience
  • Inpatient RN experience in acute or post-acute settings with medically complex patients
  • Experience in coordinated care planning and family communication
Work Environment

LTAC units operate in a hospital-based setting with an interdisciplinary team and rotating shifts. The pace is steady but can become high acuity during clinical changes.

Documentation is performed in the electronic health record, and care plans are coordinated with physicians, therapists, and case managers. Team communication and patient safety are priorities.

Evening and weekend coverage may be required as part of a rotating schedule; on-call expectations vary by facility.
