- Facilitate the RAI process under the direction of an R.N. or DNS.
- Collaborate with the interdisciplinary team to identify patient/resident needs.
- Coordinate and conduct daily case‑management meetings.
- Ensure timely MDS assessments in compliance with state and federal regulations.
- Ensure interdisciplinary team completes designated sections of the MDS assessment.
- Accurately code MDS assessments using medical record review, observation, and interviews.
- Maintain an accurate documentation system to support MDS coding.
- Track MDS assessment schedules (time frames and due dates).
- Facilitate care‑plan conferences with the team, patients, residents, and families.
- Obtain, review, and maintain all state and federal reports, making appropriate corrections timely.
- Monitor quality measures to ensure MDS accuracy and reflect data quality.
- Provide education related to the RAI process.
- Coordinate and complete electronic submission of required documentation to state databases and other entities per company guidelines and regulations.
- Ensure Medicare and Medicaid regulatory guidelines are documented accurately and timely.
- Provide managed‑care case management at the center level.
- Review additional documentation requests with the Health Information Management Coordinator to ensure document availability and compliance.
- Participate in billing reviews (Triple Check) to verify claims support by MDS assessments.
- Participate in quality assurance and process‑improvement activities.
- Continuously update knowledge base related to Medicare, managed care, Medicaid, RAI process, and technology.
- May be required to perform direct care nursing duties as needed by patient/resident and facility demands.
- Must hold a current LPN nursing license in the state of employment.
- Experience in long‑term care settings.
- Proficient computer skills.
- Knowledge of Medicare, managed care, and RAI processes required.
- Participation in the LPN to RN Back‑to‑School transition program (preferred).
