Genesis Medical Health is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its growing medical team providing primary care services to geriatric and post-acute patients within a skilled nursing facility in Oneonta, NY.

This is an excellent opportunity for a provider looking for a consistent 3-day-per-week schedule, strong earning potential, and the ability to practice independently while being supported by an established medical team.

• 3 days per week in the facility

• Consistent daytime schedule

• Participation in an on-call rotation is required

• Approximately 25–30 patients per day

• Provide primary, post-acute, and long-term care to skilled nursing facility residents

• Perform admissions, follow-up visits, assessments, and chronic disease management

• Evaluate changes in condition and address acute medical concerns

• Review labs, imaging, medications, and clinical reports

• Collaborate with physicians, nursing staff, specialists, families, and facility leadership

• Participate in the on-call schedule and remain available for urgent clinical needs during assigned coverage

• Help reduce unnecessary hospitalizations through proactive patient management

• Complete documentation and EMR charting in a timely manner

• Active New York NP or PA license

• Ability and willingness to participate in the on-call rotation

• Strong clinical judgment and ability to work independently

• SNF, nursing home, primary care, internal medicine, or geriatric experience preferred

• Comfortable managing medically complex patients

• Reliable transportation
