About the Role
As an Intake Coordinator at CareM Support, you are the front door to care — the first connection in a patient’s journey. In this role, you’ll coordinate referrals and admissions with precision and urgency, helping ensure patients are onboarded smoothly into home‑based services.
This is a fast‑paced, high‑impact position where your ability to communicate clearly, stay organized, and manage competing priorities directly supports patient care. You’ll work alongside a strong, collaborative team that relies on each other to get important work done well — and done right.

- Receive and process patient referrals via phone, fax, email, and in‑person communication
- Verify insurance benefits and communicate eligibility clearly to internal teams and referral sources
- Enter and maintain accurate patient data in EMR systems
- Coordinate start‑of‑care details, including patient outreach and physician verification
- Track documentation and follow up with internal teams to ensure timely completion
- Collaborate with clinical, billing, and operations teams to support seamless patient onboarding
- 1–2+ years of experience in healthcare intake, admissions, scheduling, or a related role (preferred)
- Strong communication skills — clear, professional, and responsive
- High attention to detail, especially with data entry and documentation accuracy
- Ability to manage multiple priorities in a fast‑paced environment without losing focus
- Basic understanding of insurance verification and healthcare workflows
- A calm, steady presence — someone who can stay organized and composed under pressure
