Description: The position serves as a Program Support Assistant to an assigned Home-Based Primary Care (HBPC) PACT team, under the supervision of the HBPC Program Director. The primary purpose of this position is to provide assistant level support to the Program Director, Nurse Manager, Assistant Nurse Manager, Medical Director and the HBPC interdisciplinary staff.
Duties- Performs general clerical and administrative duties to include preparing correspondence; meeting minutes; compiling reports; extracting and organizing data; reviewing documents for accuracy; maintaining files; and, managing and maintaining databases.
- Executes several office automation functions such as storing and retrieving electronic documents and files; formatting letters; creating and compiling HBPC weekly, monthly, quarterly and annual reports and memoranda; and, preparing materials for publications such as pamphlets, forms, graphs or technical manuals.
- Performs as HBPC timekeeper for staff including payroll software and time and leave requirements.
- Maintains a calendar and coordinates meetings (i.e., interdisciplinary staff meetings, care planning meetings) and conference calls as well as prepares materials for these events.
- Responsible for recording and typing minutes and assisting the Program Director and Medical Director with administrative functions.
- The PSA maintains equipment inventory list specific for the program and conducts annual physical inventory of equipment when requested. Generates a variety of materials for distribution including incoming and outgoing correspondence, forms, statistical reports, etc.; maintains cumulative records on program reports and responses; routine data and information requested from outside services; policy statements; confidential patient records, medications and equipment; home visits, patient status, supplies, HBPC fiscal budget, phone consultations, etc.
