Colure Health is seeking an Enrollment Specialist to be the first point of contact in the Mozaiq Care+ journey. You will identify eligible patients from discharge lists, verify insurance, obtain consent, and complete enrollment before handing off to a Care Coordinator.
This remote, ET-friendly role requires bilingual Spanish/English communication, strong data-entry skills, and meticulous records to support post-discharge care programs across TCM, CCM, RPM, and BHI.
