Highlights- Part‑time (20 hours/week) remote Hospice Physician
- Managing 2 hospice teams as a hospice team physician
- Providing leadership, clinical oversight and medical guidance for hospice staff
- Collaborating with on‑site team and reviewing appropriate goals of care
- Managing the ongoing care of patients, and monitoring progress
- Providing collaborative guidance with symptom management, medication stewardship, and controlled substance monitoring
- Working collaboratively with experienced, compassionate staff, and a team of nurses and specialists at each location
- Using EPIC EMR
- Being part of an integrated nationally recognized organization with physician‑led medical group and embedded service line
- Leveraging access to DAX Copilot, an AI‑powered clinical documentation tool integrated with Epic, streamlining clinical workflows by generating draft patient notes for efficient review, editing, and signature
- Experiencing an environment that provides safe and equitable care for all patients
- Completion of an accredited Residency Program
- Completion of an accredited HPM fellowship is preferred, not required
- If not fellowship trained, hospice or palliative experience is required
- Compassionate physician with a strong understanding of the hospice philosophy of care and expertise in hospice medical management
