Join a patient-focused, value-based primary care practice in Brockton, MA, dedicated to providing high-quality, relationship-driven care for patients age 65 and older. • Outpatient-only primary care in a value-based care model
• Patient panel capped at 750 to ensure quality care and manageable workload
• New physicians start with ~100 patients and ramp up gradually
• See approximately 18 patients per day
• Monday-Friday schedule with one remote|work-from-home day per week
• Limited call responsibilities
• Exceptional Clinical Support
• Robust care team including EMTs, APPs, Medical Assistants, and Care Managers
• Dedicated acute care support team with nurses triaging and responding quickly to patient needs
• Strong infrastructure allowing physicians to focus on patient care-not paperwork

Excellent base salary plus bonus potential
