• Deliver longitudinal primary care
• Manage transitional care post-discharge
• Reduce hospital readmissions

• Oversee care across a defined regional footprint

• MD or DO
• Board Certified/Eligible IM or FM
• Active state license (or willing to obtain)
• Comfortable with community-based care delivery
• Eligible to work in the U.S.

• Experience in home-based, ALF, or SNF settings
• Interest in regional leadership growth

• Pay: $220K–$260K base
