Home Based Primary Care PhysicianRole Overview
The Home-Based Primary Care Physician provides comprehensive primary care services to veterans in their homes, ensuring continuity of care within a multidisciplinary healthcare team. Reporting to the Chief of Primary Care, the provider is a key clinical member within the medical center.
The provider will be responsible for the assessment and care of new and returning patients in the Home-Based Primary Care program. Responsibilities include conducting home visits, developing care plans, coordinating with multidisciplinary teams, and providing follow-up care to ensure optimal health outcomes.
Responsibilities Include Veteran-Centered Care- Deliver comprehensive home-based primary care services with an emphasis on health maintenance, disease prevention, patient education, and management of chronic illnesses.
- Promote shared decision-making with veterans and their families to achieve optimal health outcomes.
- Conduct home visits to assess and manage new and returning patients in the Home Based Primary Care program.
- Develop and implement individualized care plans.
- Coordinate care with multidisciplinary teams, including nurses, social workers, and other healthcare providers.
- Participate in continuous quality improvement projects to enhance the delivery of care.
- Utilize various quality measurement tools to monitor and improve clinical performance and patient outcomes.
- Serve as a mentor and role model for other healthcare providers within the home-based primary care team.
- Engage in educational activities, providing guidance and support to residents, students, and other trainees.
- Coordinate care for veterans involving specialty care, mental health services, and other ancillary services to ensure comprehensive healthcare delivery.
- Collaborate with other healthcare providers to develop individualized treatment plans for veterans.
- Utilize the VA's electronic medical record system for documentation, ordering of tests, and communication with patients and healthcare team members.
