Lead Physician in Primary Care based in Cincinnati, Ohio, for a healthcare organization specializing in value-based care for medically complex and socially vulnerable populations. The Lead Physician serves as both a direct care provider and a clinical leader. The role is designed to break the cycle of "emergency room-centric" care by providing comprehensive, team-based medical services. Approximately 70–80% of the time is dedicated to direct patient care, while the remainder is focused on clinical leadership, mentoring, and operational strategy.
Responsibilities- Clinical Care:
Provide high-quality primary care to a dedicated panel of members, managing chronic diseases, acute conditions, and preventative needs. - Leadership & Mentoring:
Lead an interdisciplinary "pod" (team) of healthcare professionals; coach and mentor physicians, advanced practice providers (APPs), nurses, and behavioral health specialists. - Population Health:
Participate in "population health huddles" to achieve quality and utilization benchmarks and manage acute exacerbations onsite to prevent unnecessary hospitalizations. - Collaboration:
Partner with the Medical Director to shape clinical operations and workflows; represent the organization in relationships with community partners, hospitals, and payers. - Continuum Management:
Track member status across all care settings (ER, hospital, home-based) to ensure stabilized transitions of care.
- Licensure:
Active, unrestricted medical license in Ohio. - Experience:
At least 5 years of clinical experience, including experience leading interdisciplinary teams. - Background:
Experience in primary care, inpatient medicine, or community-based settings serving complex populations. - Attributes:
Strong interpersonal skills, critical thinking, and a mission-driven commitment to value-based care models.
- Sign-on Bonus:
Up to $50,000. - Total Potential Compensation:
Over $300,000. - Benefits:
Comprehensive package including 401(k), dental, health, vision, malpractice insurance, paid time off, and relocation assistance.
