Responsibilities
  • Provide comprehensive skilled services as ordered by the attending physician.
  • Educate patients and their family members and ensure patient safety.
  • Complete accurate documentation and remain compliant with all legal rules and regulations.
Qualifications & Attributes
  • Licensed Physical Therapist in the state of employment.
  • Minimum of one year of experience in an acute setting.
  • Home‑care experience desired.
  • Strong communication skills and ability to establish positive impressions with patients, families, physicians, and co‑workers.
  • Knowledgeable of therapy best practices and able to make appropriate physical therapy judgments.
  • Detail‑oriented, observant, disciplined, and able to prioritize and be timely.
  • Valid driver’s license and auto insurance with your name as a listed driver.
Major Areas of Responsibility
  • Provide skilled physical therapy in a patient’s home as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress.
  • Perform OASIS assessments to develop an individualized plan of care and adjust as needed.
  • Complete initial evaluation and admission visit within 48–72 hours of referral.
  • Develop plan of care to help decrease re‑hospitalizations, especially for high‑risk patients.
  • Treat patients to relieve pain, restore or maintain function using exercise, massage, heat, water, etc.
  • Arrange out‑patient services when not possible in the home setting.
  • Promote continuity of care with appropriate admissions, transfers, and discharges.
  • Counsel patients and families on disease/injury management at all therapy phases.
  • Incorporate patients and families in the development of the plan of care.
  • Instruct patients and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Ensure patients/families understand financial responsibility for needed equipment through the DME department or MS.
  • Teach patients on preventative measures and decision‑making to reduce re‑hospitalizations.
  • Listen to patient and family members.
  • Oversee and supervise therapy services of PTAs and, when appropriate, nurse assistants.
  • Evaluate the plan of care based on patient response and adjust as needed.
  • Communicate with PTAs and nurse aides on supervisory visit results.
  • Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
  • Complete chart audits as requested.
  • Maintain an active PT license in the state of employment.
  • Communicate with scheduler about changes as needed.
  • Participate in all on‑call requirements, case conferences, in‑service trainings, mandatory agency meetings, and orientation/training of new employees.
  • Maintain a professional appearance as a representative of the company.
  • Understand and practice agency policies and procedures.
