Harmony Cares is expanding into the South Central Pennsylvania market and is seeking a Physician Lead (Medical Pod Leader) to help launch and grow our home-based primary care practice.
This is a unique opportunity - with up to 50% of your time dedicated to leadership responsibilities - to build a new practice with the support of an established, physician-led organization that has delivered home-based primary care for more than 30 years and now operates in 14 states nationwide.
- Our model is built on value-based care, allowing physicians to focus on quality outcomes and meaningful patient relationships rather than high patient volume.
- Harmony Cares physicians and APPs deliver primary care by visiting medically complex patients in their homes, working alongside a care team that helps coordinate services, manage chronic conditions, and support patients between visits.
- Our providers see a manageable number of patients each day (typically 6-9,) allowing time for thoughtful visits, care coordination, and strong relationships with patients and families.
No patient acquisition required. Our patients come through established partnerships with health plans, allowing physicians to focus on delivering high-quality patient care. No hospital rounding.
Location:
Our team will serve patients throughout Berks, Lancaster, Dauphin, and York counties, with each provider caring for geographically clustered patient panels within their local area.

The Medical Pod Leader provides home-based primary care while mentoring and supervising a team of Advanced Practice Providers (APPs) and collaborating with interdisciplinary clinicians caring for medically complex patients.
This role blends direct patient care with clinical leadership, helping build and guide the local practice while maintaining a small panel of patients.
QualificationsMD or DO with an active Pennsylvania medical license
Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
Experience collaborating with or supervising Nurse Practitioners or Physician Assistants strongly preferred
Strong clinical experience caring for medically complex adult patients
Interest in leadership and relationship-centered care
