Position: Physician (Home Based Primary Care Medical Director)

Summary

Lt. Col. Luke Weathers Jr. VAMC in Memphis, TN is recruiting a Home-Based Primary Care (HBPC) Medical Director Physician to serve as a full-time Geriatrics and Extended Care physician. The position is accountable to the Chief of Geriatrics and Extended Care, ensuring effective delivery of state‑of‑the‑art care and compliance with standards.

Qualifications
  • United States citizenship; non‑citizens may be appointed only when it is not possible to recruit qualified citizens in accordance with VA policy.
  • Doctor of medicine or an equivalent degree in allopathic or osteopathic medicine from an institution approved by the Secretary of Veterans Affairs for the year of completion.
  • Current full and unrestricted license to practice medicine or surgery in a U.S. state, territory, commonwealth, or the District of Columbia.
  • Residency training approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. See notes on ACGME/AOA accreditation, post‑medical‑school training, or non‑U.S. residency training.
  • Residents currently enrolled in ACGME/AOA accredited programs who meet the basic requirements may be appointed as "Physician Resident Providers" (PRPs) on an intermittent basis under limited scope of practice.
  • Proficiency in spoken and written English.
  • Valid driver’s license.
  • Preferred experience:

    outpatient or in‑home management of frailty, complex chronic medical conditions, geriatric or elder care, and interdisciplinary team collaboration.

Responsibilities
  • Serve as the HBPC Program Medical Director, responsible for overall medical care delivered by the HBPC team in the Memphis VA Medical Center.
  • Collaborate with the HBPC Program Director and provide medical leadership, advocacy, and strategic direction.
  • Act as backup for other HBPC team providers and the inpatient Geriatrics Evaluation and Management unit physician.
  • Clinically supervise program staff physicians and mid‑level providers.
  • Oversee clinical supervision and management of patients, ensuring high‑quality care and performance improvement activities.
  • Monitor the appropriateness of care provided and address patient medical complaints and unusual occurrences.
  • Conduct home visits to provide care or resolve issues when appropriate.
  • Develop and coordinate educational programs for medical staff and clinical experiences for medical students, residents, and fellows assigned to the HBPC Program.
  • Participate in HBPC team meetings to discuss clinical, administrative, and policy issues, offering solutions and identifying improvement areas.
  • Keep the team apprised of advancements in medical practice and community resources.
  • Arrange primary‑care provider coverage and communicate coverage plans.
  • Collaborate with community home‑health and hospice agencies as needed.
  • Assist with administrative duties such as reviewing, approving, and signing required physician forms.
  • Remain available for collaborative problem‑solving with team members.

  • Education Debt Reduction Program (EDRP) eligibility: student loan repayment up to $200,000 for 1–5 years (subject to VHA policy). Former EDRP participants are ineligible to re‑apply.
