Form Health is a virtual obesity medicine clinic delivering evidence-based obesity treatment through telemedicine. We provide care via a multidisciplinary team including physicians, APPs, and Registered Dietitians using our proprietary app for video visits, messaging, and data sharing.
We seek Obesity Medicine physicians licensed in NY, VA, or MA to work remotely (40 hours/week). Flexible schedules, collaborative culture, and a strong focus on patient outcomes are offered.
