What You'll Do In This Role: As a Primary Care Physician at Huron Valley PACE, you'll have the opportunity to build meaningful relationships with older adults while delivering compassionate, high-quality primary care that helps them remain independent and thrive in their communities. As a valued member of our Interdisciplinary Team (IDT), you'll collaborate with a dedicated team of professionals to develop personalized care plans, improve quality of life, and make a lasting impact on the health and well‑being of every participant you serve.
Key Responsibilities Clinical Care- Provide comprehensive primary care to PACE participants across clinic, home, nursing facility, and hospital settings, including routine and change-in-condition assessments.
- Diagnose and manage acute, chronic, and complex geriatric conditions while prescribing and monitoring medications for safety and effectiveness.
- Develop individualized care plans in collaboration with the Interdisciplinary Team (IDT) and ensure care aligns with evidence-based practices and quality standards.
- Actively participate in Interdisciplinary Team meetings.
- Collaborate with nursing, social work, rehabilitation, dietary, transportation, and other team members to coordinate participant care.
- Serve as the primary medical consultant to the care team by identifying and addressing participants' medical, functional, psychosocial, and preventative care needs.
- Coordinate specialty referrals and consultative services.
- Communicate and ensure continuity of care with hospitals, emergency departments, skilled nursing facilities, specialists, and community providers.
- Review consultation reports, diagnostic studies, and hospital discharge summaries.
- Manage chronic disease programs and age‑related conditions.
- Conduct advance care planning discussions and support participants' goals of care and preventive health services and wellness initiatives.
- Participate in palliative and end‑of‑life care planning as appropriate.
- Document assessments, treatment plans, and clinical decisions according to regulatory requirements.
- Comply with CMS, state, federal, and organizational regulations governing PACE programs.
- Participate in quality improvement, risk management, and utilization review activities.
- Support organizational quality initiatives.
- Monitor clinical outcomes and participate in performance improvement projects.
- Assist in development and implementation of clinical protocols and best practices.

Skills, Knowledge & Expertise- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Current, unrestricted medical license in the State of Michigan (or ability to obtain).
- Board Certification or Board Eligibility in Geriatric Medicine and/or Hospice and Palliative Medicine, Internal Medicine or Family Medicine.
- DEA registration and controlled substance authority.
- Three or more years of primary care experience, including at least one year caring for frail, medically complex older adults; experience in PACE, managed care, home‑based primary care, or long‑term care
- Current Basic Life Support (BLS) certification.
