For more than 30 years, Harmony Cares has delivered physician-led primary care directly in patients’ homes. Today, our teams serve medically complex patients across 14 states through a value-based care model centered on quality, continuity, and outcomes.
Physicians and APPs are supported by an interdisciplinary team that includes nurses, social workers, care coordinators, and other clinicians. With a typical daily schedule of 5–9 patient visits, providers have the time to address complex needs, coordinate care, and build lasting relationships with patients and their families.
Patients are referred through established health plan partnerships, so providers are not responsible for building a patient base. There is also no hospital rounding.
Build and Lead a New MarketHarmony Cares is growing across the Lehigh Valley and South Central Pennsylvania and is seeking a Physician Lead (Medical Pod Leader) to help establish and develop our home-based primary care practice throughout the region.
This position offers a meaningful blend of clinical practice and leadership. Up to 50% of your time may be dedicated to leading the local clinical team, mentoring Advanced Practice Providers, and helping shape the practice as it grows. You will also maintain a smaller patient panel and continue providing direct care in the home.
Providers are assigned geographically concentrated patient panels within their local communities, helping reduce unnecessary travel while allowing the team to serve patients across multiple counties.

- MD or DO with an active Pennsylvania medical license
- Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
- Experience collaborating with or supervising Nurse Practitioners or Physician Assistants strongly preferred
- Strong clinical experience caring for medically complex adult patients
- Interest in clinical leadership, value-based care, and relationship-centered medicine
