Hope West is seeking a Physician (MD or DO) to join its PACE program in Grand Junction, CO. This role offers part-time to full-time hours (20-24 per week) with a strong emphasis on geriatric, home-based, and comprehensive care.
Responsibilities include providing primary care in clinic, home, and day center settings, contributing to the interdisciplinary team, and coordinating care with specialists. Board certification or eligibility in Family or Internal Medicine is preferred.
