As a Staff Physician on Clover’s Home-Based Care team, you will:

• Conduct comprehensive in-home assessments for Medicare Advantage members
• Lead post-discharge and transitional care visits
• Manage chronic conditions and identify changes in clinical status early
• Close care gaps and connect members to follow-up care, specialists, and resources
• Collaborate with RN care managers, pharmacists, physicians, and clinical partners
• Use Clover Assistant to guide proactive, evidence-based care
• Deliver compassionate care to older adults and medically complex patients

North Jersey:
Bergen, Essex, Hudson, Passaic, Union, Morris
Central Jersey:
Middlesex, Monmouth, Mercer, Somerset, Ocean, Hunterdon
South Jersey:
Camden, Gloucester, Burlington, Cumberland, Atlantic, Cape May, Sale

• Care deeply about improving outcomes for vulnerable senior populations

• MD or DO degree
• Board certification in Internal Medicine, Family Medicine, Emergency Medicine;
  Geriatrics is a plus
• Active New Jersey license, DEA, and CDS registrations
• Strong clinical judgment and experience managing chronic conditions
• Tech-savviness and curiosity, with the ability to use clinical technology, including Clover Assistant to make more informed decisions, identify care opportunities, and deliver more proactive patient care.
• Commitment to caring for older, medically complex, and underserved populations
• Home based care, value based care experience a plus
