Physician — Post-Acute / ISNP
Most physicians see the most vulnerable patients for the least amount of time. At Care More's Nevada ISNP program, that's reversed. You'll provide longitudinal, comprehensive care to a complex senior panel in skilled nursing and long-term care settings — relationships that matter, not revolving-door medicine.

- Deliver comprehensive primary care to Care More ISNP members with complex chronic conditions in SNF, LTC, and institutional settings.
- Conduct admission evaluations, routine follow-ups, and acute visits.
- Develop individualized care plans including chronic disease management and advance care planning.
- Coordinate transitions of care: hospital discharges, readmission prevention, medication reconciliation.
- Collaborate with nurses, care managers, social workers, specialists, and facility staff.
- Support STARs/HEDIS quality outcomes and reduce avoidable ER visits and readmissions.
- $211,369–$317,053 base salary + individual and company performance bonus

- In-center, home-based, and virtual care — right care, right setting, right time
- Focus on quality outcomes and patient relationships, not visit volume
- MD or DO degree; FM, IM, or Geriatrics residency;
Board Certified or Board Eligible - Active NV medical license (or ability to obtain) and valid DEA number
- 2+ years SNF/LTC/post-acute or complex senior care experience preferred
- HCC, Medicare Advantage, HEDIS, and Stars documentation experience preferred
- Comfort in institutional settings; ability to travel between NV facilities
