Home Based Primary Care Medical Director

This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until September 30, 2026. The initial cut-off date for referral of eligible applicants will be August 17, 2026 with subsequent cut-off dates every week. Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until position is filled.

Pay:

The HBPC Medical Director works under the supervision of the G&EC Service Line Manager. The physician is responsible for the overall medical care and clinical practice for the HBPC Program. The incumbent is responsible for the day-to-day operations including assignments, ensuring coverage and working collaboratively with several other service lines to accomplish the mission. The incumbent ensures the quality and timeliness of care provided, customer satisfaction, as well as compliance with standards of accrediting bodies such as The Joint Commission (TJC) and the Office of Inspector General (OIG).

The physician amends team meetings and makes home visits (Government vehicles with GPS and gas cards will be provided). The physician refills/renews medications and takes care of inter current medical problems of home bound HBPC patients, by phone or by visiting in person and ordering medications. The HBPC Medical Director is responsible for the overall medical care delivered to homebound patients in the Carl Vinson VA Medical Center service area to include Tifton Community Based Outpatient Clinic (CBOC) and Milledgeville CBOC.

Medical care includes identifying the patient's medical problems, diagnosing and treating appropriately, determining the need for consultation from subspecialty care for specialized medical, surgical or psychiatric services and determining the need for services that may require transferring patients to other VA on Non-VA healthcare facilities, community nursing homes or discharge when appropriate. The appointed physician very rarely may be detailed to cover CLC inpatient units in case of physician shortage emergencies.

The appointed physician will work as a team with nurses, social workers, nutritionists, psychologists, etc. within the HBPC team. Also, the appointed physician should be willing to teach medical students, interns, and Residents, Fellows and Physician Assistant (PA) and Nurse Practitioner (NP) students. The appointed physician will actively participate in an interdisciplinary environment that includes nurses, social workers, dietitians/nutritionists, psychologists, etc.
