• No NV state income taxWhat you'll do Deliver comprehensive primary care to Care More ISNP members with complex chronic conditions in SNF, LTC, and institutional settings.
Conduct admission evaluations, routine follow-ups, and acute visits.
Develop individualized care plans including chronic disease management and advance care planning.
Coordinate transitions of care: hospital discharges, readmission prevention, medication reconciliation.
Collaborate with nurses, care managers, social workers, specialists, and facility staff.
Support STARs/HEDIS quality outcomes and reduce avoidable ER visits and readmissions.
Compensation & benefits $211,369–$317,053 base salary + individual and company performance bonus

What you bringMD or DO degree; FM, IM, or Geriatrics residency;
Board Certified or Board Eligible Active NV medical license (or ability to obtain) and valid DEA number2+ years SNF/LTC/post-acute or complex senior care experience preferredHCC, Medicare Advantage, HEDIS, and Stars documentation experience preferred
Comfort in institutional settings; ability to travel between NV facilities
Longitudinal care for Nevada's most complex senior patients.
