Bluegrass Care Navigators is seeking a Home-Based Primary Care Physician to provide direct patient care in patients' homes and related facilities. You will supervise nurse practitioners and deliver expert clinical care for chronic and urgent conditions.
Applicants should hold an MD or DO with Kentucky licensure, a valid DEA registration, and be board certified or eligible within five years. A driver's license and reliable vehicle are required.
