We are seeking an experienced Nurse Practitioner or Physician Assistant for a Locums assignment to convert toPerm (with in 3-6 months) at a primary care practice serving geriatric patients ina value-based care model. This is a hybrid role with blend of coverage in outpatient clinics as well as home-based patient care.
Compensation: $75 - $95 perhour per day
Position Details:
- Schedule: Full Time Hours Guaranteed
- Monday - Friday - 5 Days Per Week
- 8:30 Start Time
- Patient Load: LOW DAILY CENSUS
- Home Care: 3-5 patients per day
- Clinic: Small census of 7-10 patients per day
- Setting: Hybrid (Outpatient clinic + home-based care), value-based primary care for elderly patients
Responsibilities:
- Deliver comprehensive primary care services, including preventive care, chronic disease management, and acute care to Geriatric patients
- Diagnose, treat, prescribe medications, and develop individualized treatment plans
- Order and interpret diagnostic tests (labs, imaging, etc.)
- Document accurately in the EHR system
- Provide patient education and promote wellness initiatives
- Collaborate with physicians, nurses, and support staff to ensure coordinated, high-quality care
- Refer patients to specialists and manage follow-ups
- Ensure compliance with state, federal, and professional regulations
Requirements:
- Active NP or PA license in NY
- Minimum 2 years of primary care experience
- National Board Certification
- Expedited Credentialing
- Medicaid and Medicare Provider
