As an Advanced Practice Provider in Family Medicine,you will work with a team of healthcare professionals committed to providing quality care and excellent patient  this role you willwork with 4 physicians and 3 advanced practice providers who placehigh importance on teamwork and collaboration. Our team is well-known for synergy,andhigh patient satisfaction scores.

• You will practice at Hurricane Valley Clinic in Hurricane, UT

• Your hours will befull-time,andyourschedulemay be 3 to 4 days per week depending onthe clinic’sneeds. Our clinicis open Monday - Friday 7am - 7pm weekly.

• Youron-call responsibilities areshared among all providers and will be approximately every7thweek for 7 days.

• APRN or PA-C degree from an accredited program

• Board certification

• Active,unobstructed
  
  Utah medical license, or the ability to obtain one

• Full prescriptive authority and an active DEA, or the ability to obtain one

• Ability to successfully complete Intermountain Health's credentialing process

• 2+ Years of experience is preferred

• BLS Certification

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$57.33 - $88.47
