Physician, Post Acute & Geriatric Medicine; Sacramento, CA)

Opportunity Information:

Sutter Medical Group is seeking a Post Acute & Geriatric Medicine Physician to join our expanding team in the Greater Sacramento area.

This role is ideal for physicians who enjoy caring for medically complex patients during the critical transition from hospital to recovery, and who value autonomy, continuity, and long term patient impact.

You will manage a highly sustainable daily volume designed for comprehensive care. By avoiding a high-volume production model, our physicians typically average single-digit to low-teens daily encounters (historically ~8 to 12 patients/day, including admits and follow-ups).

Schedule & Clinical Workflow

  8am - 5pm, Monday Friday schedule with high daily autonomy.

• Focused Regional Footprint:
  Care is currently localized to key partner facilities in the Greater Sacramento area. As the program expands, facility coverage is deliberately clustered to minimize daily windshield time.

• Remote Charting Flexibility:
  Once on-site rounding and initial assessments are completed, physicians are free to complete charting and administrative duties remotely. A full home-office tech suite (laptop, monitors, docking station, cell phone) is provided.

• Clinical Support:
  You will work in direct collaboration with an Advanced Practice Clinician (NP) who manages routine follow-ups and discharges, freeing the physician to focus on initial 72-hour admissions and complex assessments.

• Zero Outpatient In-Basket:
  Say goodbye to the outpatient Epic in-basket. Patient panels are managed in Epic, but communication is strictly contained to facility-level clinical workflows. (dual-charting across Sutter Epic and PCC).

• Call Coverage: 1:2 shared call rotation managed autonomously between the two physicians. Call is 100% telephone-only; physical facility visits are rare.

Compensation & System Benefits

• Compensation: $290,072 - $322,302 annually.

• Value-Based Care (VBC): 10% of the benchmark salary is tied to highly achievable VBC and quality metrics (e.g., participate in facility multidisciplinary meetings, POLST completion, and 72-hour admission access).

• Public Service Loan Forgiveness (PSLF):
  Sutter Medical Group is a PSLF-eligible organization.

• Academic & Research Access:
  For those interested in maintaining an academic footprint, robust clinical trial infrastructure and physician-led research grants are available through the Sutter Institute for Medical Research (SIMR).

• Leadership Track:
  As we actively expand this service line toward the Roseville market, this position offers a direct pathway to Medical Director responsibilities.

• Medical degree (MD or DO) from an accredited school of medicine.
• Board certification in Internal Medicine or Family Medicine (Geriatric Medicine certification preferred).
• Active and unrestricted medical license in California.
• Demonstrated experience in post-acute, skilled nursing, long-term care, or geriatric settings.
• Strong clinical skills in managing complex, multi-morbid patients.
• Excellent communication and interpersonal skills, with a patient- and team-centered approach.
• Ability to work effectively in an interdisciplinary, facility-based environment.
• Proficiency in electronic medical records (EMR) systems.

Preferred Qualifications:

• Fellowship training in Geriatric Medicine or experience in post-acute/rehabilitation medicine.
• Knowledge of CMS regulations, SNF billing and documentation requirements, and quality metrics.
• Experience participating in value-based care or population health programs.
