Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations, engaging with patients in their own homes and bridging gaps in care.
Each visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and optional diagnostic tests as ordered by the health plan. You will travel locally within licensed states and may conduct virtual visits as needed.
