Remote Work by frequent travel required: Most travel within the state of Texas
GENERAL DESCRIPTION: Link students and families with agencies and supports necessary to support independence and attainment of life goals through post-secondary transition planning. Collaborate with teachers, evaluation staff, parents and agency providers in order to meet state and federal mandates regarding person to person centered planning and futures planning. Collaborate on the vocational education classroom and work experience opportunities to students with disabilities.
Coordinate services with local school vocational programs and staff, with community, regional and state resources.
QUALIFICATIONS/ EXPERIENCE REQUIREMENTS:
- Master's Degree from an accredited college or university
- Credentials from State Licensing Agency
- Three years teaching experience in high school preferred
- Demonstrate a high degree of knowledge, understanding and skill with respect to the classroom; with exposed knowledge of post-secondary transition goal planning including education, employment, and independent living.
- Support classroom teachers in vertical alignment of transition planning.
- Collaborate in the design of vertically aligned transition curriculum.
- Monitor progress of students' mastery of Individualized Education Plan (IEP) goals and objectives that are aligned to post-secondary transition plan goals.
- Provide accurate and current progress toward meeting and/or updating the students' course of study.
- Complete and coordinate all required initial and annual transition planning meetings within timelines.
- Maintain and coordinate published information regarding adult service provider agencies, junior and community colleges, universities and other post-secondary opportunities.
- Maintain high degree of knowledge on State and Federal transition requirements.
- Ensure district compliance with all aspects of transition planning, specifically State Performance indicator 13 including entry of data in state reporting database.
- Conduct the annual follow-up survey of all special education graduates, specifically State Performance Plan Indicator 13 & 14 including entry of data in the state reporting database.
- Develop appropriate post-secondary goals, course of study and coordinated set off activities for all students in which transition planning is required by Federal or State mandate.
- Collaborate with Career Technology Education Director.
- Establish a collaborative relationship with community and agency representatives in order to facilitate the post-secondary transition requirements for student and families.
- Establish and maintain open lines of communication by conducting conferences with parents, students, principals and teachers.
- Maintain a professional relationship with colleagues, students, parents and community members.
- Use effective communication skills to present information accurately and clearly.
- Performs other duties as assigned.
Monday through Friday, during normal business hours.
