Mom’s Meals is seeking a Nutrition Educator to provide telephonic nutrition education to members with chronic conditions, helping them learn to select, prepare and eat foods aligned with their health goals. You will document goals, schedule follow-ups, and support care plans within our platform.
You will develop educational handouts, connect clients to community resources, and stay current through ongoing learning while contributing to company content and outreach initiatives.
