The base pay range is $43.30/hr - $64.95/hr this position is eligible for up to a $10,000.00 sign on bonus and subject to all taxes.

As part of a dynamic clinical team within Advocate’s vast integrated health care organization, home health offers the therapist the opportunity to work independently to assess and address the mobility, safety, communication and equipment needs of our patients.

• Assesses, plans, implements and evaluates physical therapy treatment programs established for the home care patient.
• Facilitates the progression of the patient in their home toward optimal levels of self-care.
• Ability to travel extensively within geographical boundaries in all extremes of weather.
• Works rotating weekends and holidays.

• Licensed Physical Therapist in the State of Illinois (or eligible for licensure).
• Completed 1 year of Physical Therapy clinical experience or demonstration of strong spectrum of Physical Therapy clinical affiliation experience.
• Proficient use of all home based therapy equipment and assistive devices.
• Basic knowledge of computer use.
• Ability to communicate professionally both verbally and through written reports.
