• Provide hands‑on patient care within RN scope (PICC lines, wound care, NG/G/J tubes, trach and ostomy care, IV/TPN, injections, blood draws, etc.)
• Support the Case Management RN in developing and executing patient care plans.
• Document accurately and timely within 24–48 hours, reflecting the plan of care and patient outcomes.
• Educate patients and families on diagnosis and care plans.
• Assist in skilled procedures and coordinate referrals to other disciplines.
• Maintain professionalism, privacy, and patient safety.
• Participate in on‑call duties, case conferences, and continuous growth opportunities.

• 1 year of recent nursing experience (internal or new‑grad waiver possible).
• 1 year of home‑health experience preferred; high‑acuity care an asset.
• Graduate of an accredited nursing program (Bachelor’s required).
• Current California RN license and Basic Life Support (BLS) certification.
• Valid California driver’s license with acceptable record.
• Current, compliant with all licensing and certification requirements.
• Strong critical thinking, interpersonal communication, organization, and computer literacy.
• Commitment to ongoing compliance training and certifications.

Estimated wage range: $130,000 – $160,000. Compensation follows a pay‑per‑visit model. 21
