Base pay range

$43.30/hr - $64.95/hr

PHYSICAL THERAPIST (PT) - HOME HEALTH

LOCATION: SOUTHWEST SUBURBS, Chicago Ridge, Oak Lawn, Bridgeview, Hometown, Hickory Hills, Justice, Burbank, Willow Springs, Bedford Park, Summit, Argo, Chicago , Evergreen Park

HOURS: 8am-5pm Mon thru Friday with a weekend rotation

Sign On Bonus: Up to $10,000.00 subject to all taxes. Must be employed in the exact position at time of payout. Internal team members not eligible.

Position Highlights

Physical Therapists with Advocate at Home have an enterprise with backing of a reputable, stable organization. Within Advocate’s integrated health care organization, home health allows independent assessment, mobility, safety, communication and equipment needs. Therapists serve as case managers, establishing strong relationships with patients, families and consulting with physicians and other providers.

What You Need
  • Licensed Physical Therapist in Illinois (or eligible for licensure).
  • Completed 1 year of Physical Therapy clinical experience or demonstration of a strong spectrum of experience.
  • Proficiency in all home-based therapy equipment and assistive devices (balls, bands, weights, Hoyer lifts, etc.).
  • Basic computer skills:
    Microsoft Word, email, and ability to learn computer-based scheduling and documentation system.
  • Professional verbal and written communication with strong documentation skills.
  • Good interpersonal skills, time management, and self-organization skills.
What You Will Do
  • Travel extensively within geographical boundaries in all extreme weather conditions.
  • Work rotating weekends and holidays; adhere to scheduled days and hours based on patient/client and workload assignment. Attend mandatory informational/education/branch meetings.
  • Full range of body motion required: bending, sitting, standing, climbing stairs while carrying objects, pulling, pushing, reaching, carrying, lifting (minimum 25 pounds).
  • Assist and maintain patient safety with ambulation on flat surfaces and stairs, and transferring patients using standard pivot transfers or transfer board/Hoyer lift.
  • Provide treatment on standard beds at patients’ homes; lift up to 35 pounds without assistance. Use patient handling equipment for lifts over 35 pounds, with at least one other associate when available.
  • Assess, plan, implement and evaluate physical therapy treatment programs for home care patients. Facilitate patient progression toward optimal self-care.

  • Patient relationship: develop and maintain restorative relationships throughout the continuum, per Advocate's MVP of Compassion.
  • Business development & growth: demonstrate behaviors enhancing productivity and understanding of financial/economic conditions in healthcare.
  • Patient education: demonstrate commitment to meet learning needs of patients and families.

COMPENSATION
: $90,064.00-$ annual base salary. Incentive eligible.
