Job Overview:

LHH Recruitment Solutions has partnered with a healthcare organization seeking a Social Worker. This position is based in Indianapolis, IN and requires on-site presence & occasional travel. The Social Worker is responsible for protecting and improving the social and mental well-being and functioning of patients and families. This is done by assessing patient and family needs and connecting them with community based social services and resources.

Responsibilities:

• Participate in comprehensive clinics to assess the psychosocial needs of patients and families.
• Collaborate with multidisciplinary team members to establish an appropriate plan of care .
• Consult with providers during patient office visits to provide psychosocial services.
• Educate patients and families about available community resources.
• Serve as an advocate; coordinating and connecting patients with services and assistance programs.
• Provide therapeutic psychosocial assessment, supportive counseling, crisis intervention, monitoring via telephone, email, texting, virtual, and in person interaction in the home, office, or hospital.
• Contribute to discharge planning process for hospitalized patients.
• Participate in company events.
• Monitor legislation impacting person with chronic disorders.
• Coordinate with billing and insurance departments to facilitate resolution of related issues and navigating Medicaid, Medicare, and disability systems, including application process.

Qualifications:

• 3 years of related experience in healthcare setting is required.
• Master’s Degree in social work required – open to candidates currently in Master’s program.
• Valid Licensed Clinical Social Worker (LCSW) in state of Indiana, strongly preferred;
  Valid licensed Social Worker considered if currently pursuing clinical licensure.
• Valid Driver’s License and auto insurance is required.
