Responsibilities- Support the medical team and patients as a key member of the Heart Failure Program, working closely with the heart failure clinical team to provide excellent patient care, satisfaction, and ensure that patients come first.
- Enroll, engage, and coordinate patients throughout the Heart Failure Program and other value-based care management programs.
- Oversee the remote team’s telehealth schedule. Experience with telehealth (video and/or phone) is a plus.
- Contact and coordinate patients after hospital discharge for transitional care management.
- Coordinate patient care with the remote patient monitoring care team.
- Initiate patient intake over the phone, including patient symptoms, vital signs, questionnaires, surveys, and social determinants of health (SDOH) needs.
- Provide patient-centric care by ensuring patient needs are met, including medication reconciliation, coordinating lab tests, imaging, testing, and referrals.
- Maintain and manage a patient cohort that requires frequent follow-up calls, care management, and coordination.
- Meet minimum performance metrics and goals.
- Recognize and identify patient situations that require escalation to the appropriate care team provider.
- In coordination with office support, assist the provider with ongoing health care, including:
- Maintain and update patient records confidentially and accordingly.
- Ensure medical records and lab results are in patient charts prior to appointments, and obtain missing records as needed.
- Follow prescription refill procedures.
- Collect and record updated insurance information from patients.
- Organize, plan, and deliver patient education and information to patients and caregivers.
- Must have excellent organizational skills. This role requires remote management of patients located in different practices and may include different states.
- Must be able to recognize and respond appropriately to urgent/emergent situations per protocols.
- Perform clerical duties such as processing telephone calls, voicemails, digital messages, and faxing as needed.
- Perform basic triage per protocols.
- Ensure work processes are documented accordingly.
- Participate in interdisciplinary team meetings and work regularly with team staff to ensure optimal patient outcomes.
- Perform other duties as assigned.
- Certified Medical Assistant through the National Medical Assistant Certification exam (e.g., AAMA or NCCT) within one year of issuance.
- High school diploma or GED.
Experience: At least 3 months’ experience in a prior medical office setting preferred.
Skills:
- Strong computer skills; different programs, applications, and websites will be utilized.
- Knowledge of medical terminology and medication procedures. Cardiologist experience is a plus but not required.
- Enjoy providing comprehensive healthcare, being a great team player, and building relationships with people remotely.

- Strong organizational and interpersonal skills.
- Basic knowledge of CPT & ICD-10 coding procedures.
