Position: Licensed Practical Nurse (LPN)

Benchmark Human Services is a respected leader in intellectual and developmental disabilities (IDD) and behavioral health. We work with people of all ages at home, at work, and in the community through residential programs, employment services, crisis response, day services, and more.

We are hiring a Licensed Practical Nurse (LPN) to serve as a member of the Intellectual and Developmental Disabilities (IDD) Crisis Support Home team and is responsible for health and safety of individuals in the Crisis Support Home and coordinate and provide services to those receiving services. The focus of this program is to provide time-limited crisis services that support individuals with developmental disabilities in the community.

The goal is to stabilize the individual through nursing and behavioral supports, on a time-limited basis.

Schedule:

Mon‑Fri + alternating weekends 3 pm–11 pm

Responsibilities
  • Work with the treatment team to provide direct care to individuals residing in Crisis Home based upon the individual treatment plan.
  • Use positive behavioral support strategies as described in behavior support plans or behavioral guidelines.
  • Provide medical treatments, administer medication and give injections.
  • Ensure that an inventory of medications is safely and securely maintained.
  • Help individuals with bathing, dressing, personal hygiene, transferring, standing and walking.
  • Ensure healthy meals and snacks are provided following menu plans.
  • Carry out light custodial duties and transportation as needed.
  • Measure and record individual’s vital signs such as height, weight, temperature, blood pressure, pulse, respiration, blood sugars, etc.
  • Observe individuals in Crisis Home.
  • Document and report any changes in individual’s progress, conditions, adverse reactions to medication or treatment, changes in behavior, etc. Notify appropriate treatment team members and discuss necessary action.
  • Assist with the implementation of high‑intensity services such as screening, evaluation, and assessment within the limits of the license.
  • Facilitate individual transition to other supports and/or treatment.
  • Enhance communication among staff to promote high‑quality care.
  • Review documentation for ethical and lawful billing and business practices.
  • Comply with all standards to assure the health and safety of all staff and individuals served.
  • Report any suspected abuse, neglect or exploitation to supervisor or department head.
  • Practice safety drills.
  • Attend all scheduled training and staff meetings.
  • Perform other duties as assigned.
  • Plan and participate directly in recreational, therapeutic, and training activities of the individuals.
  • Report to work as scheduled.
Qualifications
  • Valid unrestricted Georgia Nursing License
  • Certification and ongoing training in crisis intervention curriculum
  • Valid CPR and First Aid Certification
  • Experience working with individuals with intellectual and/or developmental disabilities and/or mental illness who are in crisis
  • Knowledge of DBHDD community provider standards and policies
  • Valid Driver’s License and auto insurance
  • Minimum of one year supervisory experience
  • Must be computer literate
