We’re seeking a compassionate and skilled Physical Therapist to join our team. This role is responsible for providing personalized physical therapy services to patients in their homes, ensuring safety, comfort, and improved function.
Key Responsibilities
Evaluate patients and assist physicians in developing individualized plans of care.
Provide skilled physical therapy services, including functional assessments, range of motion testing, and exercise programs.
Educate patients, caregivers, and team members on therapy protocols.
Conduct re-evaluations and adjust plans to meet evolving patient needs.
Maintain timely and accurate clinical documentation.
Supervise Physical Therapy Assistants and Home Health Aides as required.
Participate in interdisciplinary team meetings and continuous education.
Ensure compliance with agency, HIPAA, and infection control policies.
Requirements
Graduate of an accredited Physical Therapy program.
Licensed Physical Therapist in the state of practice (required).
Minimum of 2 years’ experience preferred (PT/PTA).
Current CPR certification.
Strong communication, documentation, and organizational skills.
Reliable transportation and ability to travel to patient homes.
Must be able to lift and transfer patients, adapt to schedule changes, and work occasional weekends or holidays.
