Overview

POSITION TITLE:

Team Leader/Clinician

PROGRAM:
Intensive Home Based Therapy Program (IICAPS)

Responsibilities

The staff member has responsibilities for supervision and management within IICAPS (0.5 FTE) while also carrying a small caseload of clients (0.5 FTE). The candidate is an experienced, licensed clinician who works as part of the IICAPS management team responsible for all aspects of the day-to-day management of the IHBT program. The Team Leader works closely with the other team leaders of the program, as well as the Wellmore, Inc.

VP and Director on the implementation, supervision and development of the IHBT program. Candidate supervises all functions of the assigned program and staff, including performing program and staff evaluation, record keeping, training, grant management, communication with all involved parties, personnel management and other such duties. In addition to the management responsibilities, this clinician carries a small caseload of clients and is required to meet productivity, documentation standards and uphold policy/protocol of the agency.

The candidate is assigned to carry a cell phone and responds to all calls for assistance either by telephone or in person (as clinically indicated), 24 hours a day. The candidate works flexible hours based on the needs of staff and client families.

Supervision

Supervised by:
Director of Intensive Home-Based Treatment Services or his/her designee

Supervises:
Home-based teams, staff, interns or other related staff as determined by direct supervisor

Examples of Responsibilities

EXAMPLES OF RESPONSIBILITIES associated with the Team Leader position (0.5 FTE):

  • Manage day-to-day operations of the IHBT program, including staffing, supervision, training, and policy/protocol implementation
  • Provide clinical oversight of home-based services, including assessment, treatment planning, crisis/safety planning, care management, and therapy
  • Supervise clinical teams, interns, and staff; support skill development, service quality, cultural competency, and model fidelity
  • Oversee documentation, productivity, outcome data, reporting, and compliance with funder and model requirements
  • Coordinate referrals, case assignments, outreach services, crisis coverage, and collaboration with families, DCF, and community partners
  • Performs related duties as assigned to the Team Lead position

EXAMPLES OF RESPONSIBILITIES Associated With Clinicians (0.5

FTE)

  • Provide home-based clinical services to youth and families, including assessment, therapy, treatment planning, crisis intervention, and care coordination
  • Deliver services primarily in client homes during late afternoons, evenings, and weekends, with some community-based settings as needed
  • Maintain compliance with agency and model standards, including productivity and documentation requirements
  • Collaborate with community partners to support families’ clinical and concrete needs
  • Work flexibly and as part of a multidisciplinary team
  • Other duties as assigned.
Qualifications

Must maintain strict confidentiality of Wellmore, Inc. Demonstrated knowledge and expertise in managing and supervising home-based services for children with SED, including clinical oversight and grant management. Strong understanding of clinical assessment, care management, outreach, and home-based therapies aimed at preventing higher levels of care. Must complete all required model-specific training. Ability to work collaboratively with managers, clinicians, families, and external professionals, with strong written and verbal communication skills.

Working knowledge of the greater Waterbury/Danbury/Torrington area required. Must be able to navigate challenging interpersonal situations and support staff model fidelity, including participating in home visits as clinically indicated. Bilingual (Spanish-English) preferred.

Education & Experience

Master’s degree in counseling, psychology, social work, marriage and family therapy, or a related field required, along with a full Connecticut license (LCSW, LPC, LMFT, or Psychologist). Minimum of 2–5 years post-graduate clinical experience with children, adolescents, and families (5+ years preferred). Proven experience working with culturally diverse families and demonstrated leadership skills required.

IICAPS experience preferred; other evidence-based child and family models considered. Required experience supervising professionals and paraprofessionals. Preferred experience includes individual and family therapy, home-based work, crisis intervention, attachment-focused care, and managing fidelity data and documentation compliance.

Licensure/Certification /Registration

Valid driver’s license, reliable transportation and auto insurance required.
