(Job Responsibilities)

• Outreach to patients using various communication platforms, including text messages, phone calls, and emails
• Schedule and confirm appointments with patients while maintaining electronic medical records
• Update and correct patient health records in the patient management system and electronic medical record
• Provide portal support, including patient outreach and enabling access
• Run regular and ad hoc reports related to patient engagement activities

• Minimum of an Associate's Degree or Medical Assistant Certificate with one year of experience
• Strong customer/patient service experience
• Computer literacy and proficiency in using related software applications
• Excellent oral and written communication skills
• Remote workers must have access to reliable high-speed Internet and reside in the United States

• Bilingual in Spanish
• Experience with patient engagement platforms such as Centricity and eClinical Works
